Care Coordinator

Responsibilities

• ● erational, administrative and project management support to the assigned Orthopaedic Surgery Clinical Programmes.

• ● To support the implementation, development, and management of the assigned clinical programmes.
• ● To establish workflow and guidelines, and hospital policy document as necessary.
• ● Care Coordination
• ● To coordinate care across the various care settings as a programme care coordinator, and act as patient advocate between patients, caregivers, and hospital (including Telephonic Follow-up Discharged Care).
• ● Project Management
• ● To support the development and management of new initiatives or programme improvement projects that enhance clinical outcomes and patient experience.
• ● Appropriate documentation
• ● To ensure all information are documented in hospital systems after each patient's encounter for future audits.
• ● To organise regular multidisciplinary team meeting on case discussion and ensure appropriate documentation.
• ● To provide programme secretariat support and manage all programme documentation and meeting materials in hospital systems.
• ● Report and data management
• ● To support data management, data validation (including data analysis), and timely report submissions as assigned.
• ● Programme Education & Publicity
• ● To assist and support the programme education and publicity events/ activities, in close collaboration with internal and external stakeholders.
• ● Good Communication
• ● To demonstrate good communication skills in patient care delivery.
• ● To maintain good communication with work effectively with key internal and external stakeholders.
