As a Tailored Support Coordinator, you'll play a meaningful role in supporting tenants and household members to access the services and supports they need to achieve their self-identified goals and sustain their tenancies. Working closely with tenants, community services and sector stakeholders, you'll build trusting relationships, identify strengths and needs, co-design tailored support plans and facilitate access to relevant services through warm referrals and advocacy. You'll also monitor progress, support community engagement and help tenants build wellbeing, social connection and self-management.
