Responsibilities

  • Understands NDIS legislation, price guide, and funding categories.
  • Accurately interprets participant plans, goals, and budget allocations.
  • Explains plan components clearly to participants and families.
  • Identifies risks, gaps, or misaligned supports within a plan.
  • Manages scheduling or service delivery and resolves any issues
  • Demonstrates a strong understanding of how plan transitions between providers work and confidently manages the full end-to-end process to offboard from an existing provider and onboard to a new one.
  • Sources appropriate providers based on needs, goals, and preferences.
  • Facilitates service agreements and bookings confidently.
  • Ensures providers understand participant needs and expectations.
  • Coordinates multidisciplinary teams effectively.
  • Ensures the correct support workers are aligned to the participants needs.
  • Adjusts support as needs change
  • Monitors spending across Core, Capacity Building, and Capital budgets.
