Responsibilities

  • Mentor junior staff and ensure their abidance to going rules and policies as applied and implemented in the department.
  • Maintain sufficient inventory of material supplies and equipment for performance of duties.
  • Report shortage of materials to immediate supervisor in a timely manner.
  • Review medication orders and patient medication profiles for appropriateness of therapy.
  • Verify and monitor these medication orders for drug interactions, duplication and omission therapy, allergies, adverse reactions, and recommend appropriate formulary alternatives as needed.
  • Inform physicians of drug unavailability and assist in suggesting alternatives.
  • Responsible for reviewing and interpreting medication orders' for clarity, accuracy, rationality, compatibility, feasibility and suitability for the patient, when applicable.
  • Calculate and confirm pharmaceutical dosage calculations (paediatric doses, IV admixtures, TPN, chemotherapy, etc)...
  • Prepare and compound medications as per applicable standards.
  • Perform double checks on the prepared medications.
  • Comply with federal and local rules and regulations in regard to pharmacy activities, including but not limited to preparation, labeling and delivery of medication and other pharmaceutical supplies.
  • Follow appropriate procedures for dispensing medication prescriptions.
  • Maintain the required records of dispensing.
  • Provide patient counseling and advice as necessary on the proper and safe use of medication.
  • Maintain proper documentation on patient medication profiles or prescriptions.
  • Ensure and maintain medication proper storage conditions.
  • Perform inspections of nursing units to assure safe and secure storage.
  • Comply with the safe and competent use of all pharmacy equipment used in their area of practice.
  • Operate and maintain pharmaceutical equipment according to policies and procedures.
  • Conduct checks and calibration of equipment as required. Recognize, report and advise on discrepancies in performance and initiate remedial action in the light of this information. Ensure that the equipment is used in the correct manner to obtain optimal results. Inform supervisor of equipment malfunction and/or need for repair.

Requirements

  • Participate in training and education sessions for department staff and other healthcare professionals in the safe and secure handling of drugs and equipment, prescribing (whenever applicable), administration and treatment and management i.e. teaching, induction, peer review, case studies, and clinical presentations.
  • Participate in department programs/committees, various hospital committees and staff meetings as assigned.
  • Participate in the orientation and training of new employees.
  • Ensure appropriate internal and external sharing of information based on governing policies and procedures.
  • Maintain professional knowledge by attending lectures, seminars or on-line education units.
  • Complete and maintain all regulatory requirements including: licensure and certification and other mandatory training within established time frames.
  • Keep abreast of all developments pertaining to pharmaceutical care and other specialty related matters.
